Important Clinic Policies:
If your child has not been seen for an appointment with your Pediatrician within the last 12 months, a new referral will be required to reactivate your child’s medical chart.
Outdoor shoe removal is required to keep the clinic clean for your family. Consider bringing your own clean indoor footwear such as slippers for your office visit.
If you fail to show for your appointment or if you are more than 10 minutes late for your appointment or if you fail to cancel your appointment a minimum of 24 hours prior to your appointment time, you will be required to pay a fee prior to being rescheduled. If you do not receive a reminder for your appointment, you are still responsible to attend, reschedule or cancel your appointment.
